You should attempt to maintain essentially the same system operating configuration on each computer.
There may be some computer that is special or unusual and it may be configured different than all others. However, try to make the look, feel and layout of all of the computers be the same since it is likely that the users will move about the clinic and need to use more than one computer during the course of the day's work.
You may want to restrict or limit the extent to which you permit the staff to customize the desktop in Windows 95 or 98, for instance.
We prefer to maintain similar hardware as well so that maintenance issues are less complex.
